Includes index, and has 4,143 pages. No date specifically inside, but "1934" was written on the bottom of the spine.
Covers are very worn, with extreme wear on edges and corners
, discolorations, scratches. Inside pages are in very nice condition, though; albeit a few small discolorations and a couple of  pages in the back being wrinkled (see 16th photo). Some smudges visible on page edges when book is closed. Still overall a great reference item in readable condition.
